Your Privacy, Protected Like Your Health
How this website and our clinic collect, use, share, and safeguard your information — written in plain language, in accordance with HIPAA standards.
Effective date: October 23, 2026 · Accident and Injury Care, 1916 23rd Ave S, Ste B, Seattle, WA 98144
Accident and Injury Care ("we," "us," "our") is a chiropractic and injury rehabilitation clinic in Seattle, Washington. Because we provide healthcare services, we treat the information you share with us — both on this website and in our clinic — with the care required under the Health Insurance Portability and Accountability Act (HIPAA) and Washington State law. This policy explains what we collect, why, and the choices you have.
1. Information We Collect
Information you give us on this website. When you complete an appointment request, contact message, referral, or chat inquiry, we receive the details you type in — typically your name, phone number, email address, and whatever you tell us about your injury, accident, insurance, or claim (for example, your PIP claim number or attorney's name).
Information we need for your care. When you become a patient, our clinic collects the clinical and billing records described in our Notice of Privacy Practices — exam findings, treatment notes, imaging, insurance and claim documentation.
Basic website visit information. Like most websites, our site collects standard technical data (pages viewed, approximate region, device type) through the tools described in section 7.
2. How We Use Your Information
We use website submissions only to help you: responding to your questions, scheduling appointments, verifying insurance benefits, coordinating PIP, L&I, or attorney-lien billing, and improving the content of this site. We do not sell, rent, or trade your information to anyone, and we never use your health information for advertising.
3. HIPAA & Your Protected Health Information
As a healthcare provider, we are a "covered entity" under HIPAA. Information that identifies you and relates to your health, care, or payment for care is Protected Health Information (PHI). HIPAA permits us to use and disclose PHI for three core purposes, always under the "minimum necessary" standard:
- Treatment — coordinating your chiropractic, massage, and rehab care with the providers involved in your recovery.
- Payment — billing your health plan, auto PIP insurer, or Washington L&I, and processing liens or self-pay.
- Healthcare operations — quality assurance, recordkeeping, and the business functions of running the clinic.
Any use beyond these — such as most disclosures to your attorney or for marketing — requires your written authorization, which you may revoke at any time. HIPAA also gives you rights over your PHI, including the right to access and receive a copy of your records, request amendments, receive an accounting of disclosures, request restrictions on uses you object to, request confidential communications, and file a complaint with our Privacy Officer or the U.S. Department of Health & Human Services without retaliation. Our full HIPAA Notice of Privacy Practices is available at the front desk and on request — ask any staff member.
4. How We Share Information
We share your information only as needed to care for you and run the clinic: with your treating providers, your insurers and claim administrators (including auto PIP and L&I), your attorney when you have authorized a lien or representation, and trusted service providers who help us operate under signed confidentiality agreements. We also disclose information when required by law, such as a valid court order or legally mandated public-health reporting.
5. Website Forms & Email
Form submissions travel over an encrypted connection and are saved in an access-controlled database, then routed by email to our scheduling staff through our HIPAA-aligned Google Workspace email. Only staff who need the information to help you can see it. When you include your email address in a form, our staff can reply to you directly from that message. Please remember that ordinary email is not fully encrypted — avoid sending highly sensitive details (like images of your ID or full medical records) through this website, and never share information you are not comfortable sending.
6. The Chat Assistant
Our on-site chat assistant answers general questions about our services, insurance, and scheduling. Your questions are logged so we can improve the assistant and our website content; we do not use them for advertising. The chat is not for emergencies — if you are in pain or have been injured, call or text (206) 362-3344 or book an appointment so a provider can properly evaluate you.
7. Cookies & Analytics
This site uses Google Analytics to understand, in aggregate, which pages are helpful and which need improvement — for example, which insurance questions people look for most. These cookies are used only for site analytics, never for advertising or tracking you across other websites, and no analytics data is combined with your medical records. Google reCAPTCHA also runs on our forms to prevent spam and fraud. You can block or delete cookies in your browser settings at any time; the site will still work.
8. Data Security & Retention
We protect website and patient information with administrative, physical, and technical safeguards appropriate to its sensitivity: encrypted connections, role-based access to records, secure hosting, and staff training on privacy obligations. We keep website submissions only as long as needed to follow up with you and meet recordkeeping requirements; patient records are retained per Washington State medical-record laws.
9. Third-Party Links
Our site links to outside resources such as Google Maps for directions. Once you leave our website, this policy no longer applies — we recommend reading the privacy policy of any site you visit.
10. Children's Privacy
This website is intended for adults seeking injury care. We do not knowingly collect information from children under 13 through the site. A parent or guardian may contact us at info@autoaccidentcmt.com to request removal of any information submitted by a child.
11. Changes to This Policy
We may update this policy to reflect changes in our practices or the law. The "effective date" at the top of this page always shows the current version, and material changes will be highlighted on this page.
